Who We Are

About
Community Roots

Community Roots provides home and community based services for individuals in the I/DD community. We believe every person deserves dignity, meaningful connection, and the opportunity to grow independence within their community.

We don’t just provide services — we cultivate relationships that help individuals grow strong roots and experience real belonging.

Services

What We Offer

Finding the right support matters. We walk alongside families and referral partners to ensure habilitation services are individualized, compassionate, and aligned with each person’s goals.

Respite

Respite care gives families and primary caregivers a well-deserved break while their loved one continues to receive attentive, personalized support in a safe and engaging environment.

Personal

Care

Personal care provides compassionate, hands-on support for daily living — from getting ready for the day and meal preparation to household tasks and medication reminders. Our trained staff are there to help with the moments that matter most, so individuals can remain comfortably and safely in the home they love.

Employement

Services

Work is about more than a paycheck. It brings purpose, independence, and a sense of pride. Our Employment Services support individuals in finding and keeping jobs in their community that match their strengths, skills, and interests. We work alongside each person and local employers to create a good fit for everyone.

Community

Engagement

Everyone deserves a place to belong. Our Community Engagement services help individuals discover and take part in the activities, groups, and events in their own community. We start by getting to know each person: their interests, their goals, and what they’d like to try. Then we help them build real connections, whether that’s joining a club, volunteering, taking a class, or finding a favorite spot in the neighborhood.

Community

Inclusion

At Community Roots, we believe people with intellectual and developmental disabilities should be seen, known, and included in everyday community life. Our Community Inclusion services help individuals take part in the same places and activities as their neighbors, with the support they need to participate fully. We focus on building skills, relationships, and roles that help each person feel connected and valued.

How do you respond when someone becomes frustrated or has challenging behaviors?

Our focus is on safety, empathy, and proactive strategies. CR emphasizes prevention and understanding triggers. We practice de-escalation techniques and utilize calm communication, avoiding punishment approaches or power struggles.

How do you support someone to be involved in their community?

This is one of our core priorities, that is "connecting" with community. We believe deeply in the power of community connection for the health / well being of clients and for their growth as people. Our DSPs and organization strongly encourage participation in activities, hobbies, and volunteering. We also focus on fostering meaningful participation in activities, not just supervision.
